Laser skin resurfacing (CO2 fractional laser) - Laser therapy

How does skin resurfacing work?

The treatment is performed with fractional CO2 laser. Within one impulse fractional laser emits numerous microbeams which create narrow, deep columns of tissue coagulation that penetrate well below the epidermis and into the dermis. The laser impulses deliver light in an array of high precision microbeams which produce micro damage on the treated surface. The damage is intentional as it initiates a natural healing process that stimulates the formation of new skin. Each microbeam of the skin damaged with the laser is surrounded with healthy tissue which start intensive multiplication. At this point, new healing processes start to fill in the damaged treatment areas.

The aim of skin resurfacing is to damage the skin intentionally, in a planned and predictable manner. As a result the skin is stimulated to regenerate the planned damage which eventually results in a new, improved structure of the tissue.

Do I need local anaesthetic?

The procedure usually requires the application of anaesthetic cream (Emla) which is applied 30-40 minutes before the treatment. We also advise that the area is cooled just after the treatment which brings discomfort and pain to minimum.

Who is the treatment for?

The procedure is designed for both women and men who have experienced decreasing condition of their skin and wish to treat sun damage. These changes may also include wrinkles, creases, the loss of firmness and uneven pigmentation which are major indications for the treatment. Other indications may also include acne scars and enlarged pores. The treatment can be performed on any part of the body but the most commonly treated areas include the face, neck, décolletage and the hands where the skin ageing processes are the fastest due to sun damage. This procedure is also ideal for parts of body such as the neck, décolletage, eyes or the lips which are difficult to rejuvenate with other available treatments.

What are the results?

During the treatment the laser destroys the old and stimulates it to regenerate the planned damage which eventually results in a new, improved structure of the tissue. As a result the skin is stimulated to regenerate the damage which results in a new, improved structure of the tissue. Wrinkles, folds and scars become flat, lighter, while pores are minimized. The result is a smoother, clearer and more youthful appearance to your skin.

When do I see the final results of the treatment?

It is usually not until 7 days before skin heals after the treatment. However, it is too early to expect the final results at this point as the process of skin regeneration normally takes several weeks. The final result can be enjoyed after 6-8 weeks after the treatment. Another skin resurfacing treatment which is done at this time helps to stimulate skin for continuous regeneration.
How many treatments do I need to see satisfactory results?

The number of treatments vary from 1-6 depending on patient’s individual features such as aging processes, age and depth of wrinkles, pores, and acne scars. Mild wrinkles can be reduced with a single treatment while deep creases and acne scars may take 4 – 6 procedures each performed every 6-12 weeks.

How long do I see the results of the treatment?

The outcome of the treatments, which is increased quality of your skin, results from skin-deep changes to its structure and can last as long as a few years. Such evident changes can be observed with acne scars where the long lasting results are impressive. We should remember, however that such treatments do not prevent our skin from getting old completely. With time, wrinkles and the lack of firmness come back and unhealthy sun exposure gradually contributes to uneven pigmentation.

How often do I need to repeat the treatment?

First sessions which do not normally guarantee immediate results should be repeated every 6-12 weeks. After the series of 1- 6 sessions which considerably improve the condition of your skin, the procedure will not have to be repeated until a few years later.

Contraindications include

  • Pregnancy and breast-feeding
  • Oral retinoid medications
  • Herbs: St. John's-wort, marigold, horsetail
  • Photosensitive drugs : antibiotics and antidepressant medications
  • Acute skin inflammation
  • Photodermatosis (sun sensitivity)
  • Psoriaris
  • Dermal filling with hyaluronic acid (treatment can be performed after 4 weeks)
  • Injectable poly-L-lactic acid (Sculptra)- (treatment can be performed after 12 weeks)
  • Tanned skin
  • Chemical skin pills
  • Diabetes
  • Weakened immune system
  • Susceptibility to keloids
     

How do I look just after the treatment?

Shortly after the treatment you should expect the skin to be a little swollen, red and burning. In some cases dripping of tissue fluid or blood may occur. Immediately after the treatment soothing gel and saline compresses are applied to bring discomfort to minimum. 2-3 days later swelling and redness resolve and a large thin scab is formed all over the treated area. The scab will slowly be peeled of within the following 4-5 days to reveal new skin. Skin regeneration after the treatment is a continuous process which takes place deep down in the skin layers

Before the treatment: guidelines

A few days before the treatment patients are advised not to have any kinds of exfoliating treatments such as microdermabrasion or chemical peels. They should not use any creams which contain retinoids or acids. Patients should make sure they are in good physical condition.

After the treatment: guidelines

It is important to apply a soothing cream (Cicalfat, Zinalfat, Cicaplast Baume B5, Sensicure) a few times a day for 7-10 days after the treatment until the scab is removed. During this time patients cannot go to swimming pools, touch the new skin with dirty hands or try to scratch the scab off. When the scab is finally peeled off, you can apply moisturizing and fatty creams (Cetaphil MD, Terproline) and eventually resume your usual skin care practices. It is also important to use sunscreen for six months after the treatment especially in spring and summer seasons.

How can I keep the result for longer?

Patients are advised to use high factor sun protection that will protect against both UVA and UVB radiation, products rich in retinol and peptides and vitamin C. Patients are also advised not to smoke.
The whole range of skin rejuvenating treatments such as medical skin peels and mezotherapy are also highly recommended every 6 or 12 months as they will help to keep the skin moisturized, delay the aging process so that you can enjoy the results of the treatment for longer.

Recommended supplementary treatments

Skin resurfacing treatments performed with fractional CO2 laser can be considered as procedures aimed at stimulating skin for better regeneration and can be supplemented with other treatments. After the series performed with CO2 laser we recommend that our patients follow skin rejuvenating therapy with hyaluronic acid (Juvederm Hydrate, Ialest, Newest) revitalizing cocktails (CytoCare, StretchCare, NCTF, NCTF HA, Plinest, UpGrade, Fixer) or platelet rich plasma injections (Regeneris). We also highly recommend deep creases and wrinkle correction with hyaluronic acid (Restylane, Juvederm), calcium hydroxyapatit (Radiesse) or an anti-aging facial treatment (Sculptra).
